It's amazing what people that are looking out a slit or a periscope, probably dirty and through smoke, don't see during the stress of combat. I think it simulates reality pretty well.

In a combat situation, if there is little time to check on things, tighten lugs, etc, then the problems are going to seem sudden and unpredictable, but you can bet they were the result of cumulative occurences. It's like any mechanical device. It may function perfectly until the day it doesn't, but chances are whatever happened could have been predicted and possibly even prevented with enough oversight.

Definitely correct if by repair, you mean the time it takes to bail out, hoof it to the rear, and discuss recovery and repair at a much later date.
