CM will use any refresh rate that the video driver says is available. CM will typically start at your current desktop resolution when you first start it (giving you a info box on the screen listing the resolution, the refresh rate and possibly which adapter/video card you're running on). The refresh rate that is initially offered may be much higher than your current refresh rate and your monitor may have a hard time displaying it (the screen may appear 'shredded' or 'multiplied', etc.).

Basically you can let this resolution/refresh rate selection process continue unhindered (i.e. - don't press any keys) and it may eventually display a resolution and refresh rate that works on you monitor, assuming that this is the problem that you're having. You can select this resolution and refresh rate by clicking on 'Select' otherwise you can click 'Skip' or CM will automatically try the next lower refresh rate or resolution. When CM steps down to a lower resolution it will offer it at the highest refresh rate available to the video driver, so you may go from 1024x768 at 60Hz to 800x600 at 120Hz.
