Campaign quibbles

How do operations determine where the new lines will be, and which units will exist? I notice losing a few living units, and being pushed back even where the enemy made no advances, while the enemy seems to retain ground. Some new map space was added on my side of the map.

Are the units being taken away because the scenario designer somehow specified they wouldn't be in certain later scenarios?

Is the determination of where the sides will be allowed to set up something that players can predict, or that operation designers can influence?
