The front end (change pages) of the study called Evolution of the Armored Infantry Squad, Appendix G, shows exactly who goes where, on what tank, when Armored Infantry is operating in this mode. The study itself is sweeping in scope, since it shows  the prior related history down through the ages.

My guess is that they may have trained in this manner, and in practice attempted to mount up in this manner, but in the chaos of battle would have hopped on wherever there was a space available in whatever order came about. Still, it is a good example of the military trying to impose as much order and discipline as possible on what is inherently a very disorderly process.
