I think maybe the obr. 2016 version has been seen with one unit on parade, and only just this year.

    T-72B3 obr.2016  production ≈ 150 per year.

The version with CITV (“B3M”) still hasn’t made an appearance in service, only in tank biathlon.

    T-90M ≈ 60 (2018-2019).

pics of what happens with but a single turret top direct hit from a 120 mm class hit. The (forget exact model; T:64?) tank was repairable but was out of the fight, since the commander's station (right next to the hit) was wrecked, with a great gouge torn in the steel of the turret.

It is a call for companies to submit proposals for procurement.

There is only one company producing Izdelie 170-1(КТРВ)  and "call for companies to submit proposals"(ended by 28.09.2015 10:00) just a part of bureaucratic procedure.

The 42nd MRD appears to use a mix of T80Us and T90As, in my opinion T80U usage is a stopgap solution for the time being.

“The hardest thing of all is to find a black cat in a dark room, especially if there is no cat.”

42nd MRD is formed from 8th, 17th and  18th Motor Rifle Brigades and use T-72B3.
"На полигоне Гвардеец экипажи танков Т-72Б3"

TV Zvezda use “stock footage” to illustrate exercises when they don't have a correct one from this unit. In this case they used 4rd TD (only unit with T-80U) and 2nd MRD (T-90A).

Are they vehicle penetrated and burned, or is it simply the tank mobility killed, crew bailed out, and it's now behind German lines or even simply was declared a loss because it's easier to get a new tank than whatever complicated assembly fell out the back of the tank?
I'm getting caught up on my Eastern Front stuff, and lot of T-34 losses early war had everything to do with a lack of recovery assets/poorly trained crews/automotive reliability issues and very little to do with enemy fire.

Статистические данные по безвозвратным потерям танков и их членов экипажей танковых частей 5 танкового корпуса в Двинской операции (18-28.07.1944)/Statistical data on irrecoverable losses of tanks and crews in tank units of 5th Tank Corps during Dvina (Rēzekne-Dvina) operation. This was successful offensive operation. Most of the losses a from 7.5 and 8.8 cm guns.

Further in terms of Grozny you're looking at very few penetrations, and a lot of vehicles being mobility killed, and then abandoned as their panicking overwhelmed crews abandon them.  The killer of armor there was not catastrophic vehicle ruining, it was after 19-22 non-penetrating RPG-7 hits, the vehicle is untenable and was left behind.

3rd Tank Battalion, 6th Tank Regiment, 90th Tank Division.

22 tanks have penetrating hits from RPG/ATGM/Tank and AT-guns fire:
    5 total loss T-80B №198, T-80BV №174, 186, 193, 195.
    11 abandoned T-80BV №172, 177, 178, 179, 180, 181, 183, 185, 189, 191, 197.
    5 withdraw under their own power T-80B №188, T-80BV №170, 187, 190, 199.
    +1 abandoned T-72  taken by T-80BV №180 crew.
    6 crewmembers died in tanks.
    Sources (Russian):1,2.

  7.  however T-34s had a 75% mortality rate for crewmen on struck tanks let alone WIA).

    Myth.

    186520_original.png

    5th Tank Corps losses 18-28.07.1944.
    T-34 - 28/T-34-85 - 89. Crew casualties ≈1.3 KIA per tank.
    M3c (M3 Lee) - 13. Crew casualties ≈2.5-3 KIA per tank.
    IS-2 - 11. Crew casualties 1 KIA per tank.

    Another example:

For example Polish 4th Armoured Brigade had got 65 tanks (T-34) and 325 members of tank crews before the battle of Bautzen (16.04.1945). Irrecoverable losses until the end of the battle (01.05.1945) were 30 tanks. Irrecoverable losses of tank crews (not only KIA but also MIA & POW) were 55.

For T-64/72/80/90 numbers around the same (1 KIA per tank). In Grozny 30 tanks lost, crew casualties 31 KIA.

The next question is if the Malakhit is or is planned to be fielded by the Russian Army.

KM-15 "Malakhit" (development completed in 2005) is part of Krasnopol/Gran/Kitolov Guided Weapon Systems.

The 1D26 LLDR also has a thermal sight, but again I don't know if it has been adapted into service.

    LTsD-3 Atoll/1D26 (accepted into service in 2004) has only night sight but LTsD-3M/1D26M, LTsD-3M1 and LTsD-4(-1/2/3) Vizir/1D29 have thermal sights (LTsD-2M, LTsD-3M, LTsD-4, LTsD-4-1, LTsD-4-2, LTsD-4-3, KDU-1 and 9V9003 accepted into service in november 2010).

  9. Ok, let Svintets-2 remains for T-90AM only and give to this tank bigger rarity. No any information that Russia launched mass producing of this ammunition, since in priority now Armata with its Grifel APFSDS.

    3БМ-60 «Svinec-2» in production since 2013.

  10. Where did the 120mm hit?

Turret roof near the gunner's hatch.

How do you know it was a 120mm round? What kind of 120mm round was it?

Because ukrainian soldier who made this photo said so. And this is not that many options:
82mm - probably don't have enough force to deform the turret roof.
120mm 2B16/2S9 wasn't used in this area at that time.
122/152mm - will have different angle of impact.
So 2B11 120mm mortar with 3ОФ34(3,43 kg HE)/3ОФ36(3,16 kg HE) or 53-ОФ-843Б(1,4 kg HE) is most likely candidate.
Of the visible damage, how much was due to the (putative) 120mm round, how much due to post-impact fire? Was any caused by crew demolition; other weapons?

AFAIK there was none "post-impact fire/crew demolition/other weapons". Crew rest nearby when tank was hit, they drive it to the rear but turret was "total loss".
  11. All T-80 is meant to be out of service by the end of the year.

На данный момент наша дивизия переходит на перевооружение… Танк такой «Т-80Е», который усовершенствует систему управления огнем, стоят тепловизоры уже для ведения огня и днем, и ночью.
  12.

The future of T-80s in RF Armed Forces is still a bit ambiguous. A recent interview by a commander of 2nd Guards Tomanskaya Division seemed to indicate that they are expecting to get latest upgrades of T-80s in a near future... Still though, I don't believe that such upgrades have been fielded (or even presented) so far..

However, until there is some sort of official announcement
